Completing an Undergraduate Certificate Program

If you have completed a certificate program, you must fill out a Certificate Petition Form and submit it to Online and Continuing Education in order to have the certificate formally noted on your transcript. We would also like to request that students completing a certificate program also take a couple of minutes to complete the Degree/Certificate Completion Survey so that we can collect your feedback on the certificate program for purposes of quality control.
