Registration InformationREGISTRATION FORM (pdf format) Once registrations have been processed, registrants will be sent a letter of confirmation. Please allow up to 1 week for processing. Registrations must be received by October 16, 2009 along with a check, a copy of your purchase order, or credit card information. We accept VISA, MasterCard, or Discover. We cannot accept American Express. The company name, address, fax and phone number, and Federal Tax ID number must be included with all purchase orders.
